Job Description

Deliver skilled, compassionate care directly to patients homes as a Registered Nurse specializing in home health. This full-time, 13-week contract role involves field-based visits, providing clinical support and empowering patients to achieve the best possible outcomes. Self-motivation, clinical excellence, and top-notch organizational skills are essential in this role, as you will be responsible for overseeing patient care independently while collaborating with a multidisciplinary team.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (typically 35 patient visits per day)

Duties and Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized plans of care.
  • Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, note changes in condition, and promptly communicate findings to the care team and physicians.
  • Educate patients and families on treatment regimens, disease management, and preventive health.
  • Maintain detailed, accurate records using Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and home health aides for a coordinated care approach.
  • Actively participate in case conferences, staff meetings, and continuing education activities.
  • Adhere to clinical and documentation standards as required by regulations and agency policies.

Required Qualifications:

  • Active RN license (state of practice), in good standing
  • Minimum of one year of recent home health field experience
  • Demonstrated proficiency with OASIS and Home Care Home Base (HCHB)
  • Reliable transportation, valid drivers license, and auto insurance
  • Strong assessment, communication, and organizational skills

Preferred:

  • Bachelor of Science in Nursing (BSN)
  • Experience managing 2532 point caseloads
  • Familiarity with Medicare and compliance standards
