An opportunity is available for a skilled Home Health Registered Nurse (RN) to deliver high-quality nursing care directly in patients homes. This full-time, 13-week contract position involves providing patient-centered care while working independently within a structured framework of clinical accountability. The role offers a balance of fieldwork with the support of a collaborative healthcare team.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop personalized care plans tailored to each patients needs.
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, detect changes in condition, and communicate effectively with physicians and interdisciplinary teams.
- Educate patients and families on disease management, medication regimens, and preventive health measures.
- Maintain timely and accurate documentation using the Home Care Home Base (HCHB) electronic system.
- Collaborate with therapists, social workers, and home health aides to ensure coordinated care delivery.
- Participate in case conferences, staff meetings, and ongoing education to maintain clinical excellence.
- Comply with all regulatory and agency standards for patient care and documentation.
Qualifications / Desired Experience:
- Active RN license in good standing within the state of practice.
- At least 1 year of recent home health nursing experience; preferred field experience.
- Proficiency with Home Care Home Base (HCHB) electronic documentation system.
- Experience utilizing OASIS for assessments.
- Strong clinical judgment, organization, and communication skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to manage a flexible schedule and work independently.
- BSN preferred.
- Experience managing 2532 patient care points weekly.
- Familiarity with Medicare and home health regulatory standards is a plus.
Location & Work Schedule: Position is based in Durham, NC. Work hours are Monday through Friday, 8:00 AM to 5:00 PM, with daily travel to patient residences within an assigned territory. Mileage reimbursement is provided.
