A highly skilled and compassionate Registered Nurse is needed for a full-time, 13-week contract assignment in home health. In this field-based role, youll provide patient-focused skilled nursing care directly in the home setting, promoting continuity and quality outcomes. This position requires strong clinical expertise, exceptional organizational skills, and confidence working independently while serving as a vital connector between patients, families, and interdisciplinary teams.
Schedule & Assignment Details:
- Full-time contract, 13 weeks
- Monday Friday, 8:00 AM 5:00 PM
- Field-based visits to patient homes (mileage reimbursement included)
- Caseload of 2532 points weekly, typically 35 patient visits per day
- Electronic documentation via Home Care Home Base (HCHB)
- OASIS assessment tools mandated
Essential Qualifications:
- Active RN license (in good standing)
- At least 1 year recent home health experience
- Proficiency with HCHB and OASIS documentation
- Strong clinical assessment, communication, and time management skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to manage a flexible, independent, field-based schedule
Preferred Experience:
- BSN credential
- Prior experience efficiently managing caseloads of 2532 points weekly
- Familiarity with Medicare and home health compliance requirements
Key Responsibilities:
- Complete comprehensive OASIS assessments, develop and implement individualized care plans
- Deliver skilled nursing care: wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress and collaborate with care teams for optimal outcomes
- Provide clear patient/family education on conditions, medications, care plans, and prevention
- Maintain accurate, timely documentation and uphold regulatory standards
- Engage in case conferences and ongoing professional development
