Job Description
Provide skilled nursing care to patients in their homes as a Home Health Registered Nurse (RN), ensuring exceptional clinical care and patient-centered service in a flexible, field-based role. This full-time, 13-week contract position offers a Monday through Friday schedule from 8:00 AM to 5:00 PM with daily travel within an assigned territory. Mileage reimbursement is provided.
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate promptly with care teams and physicians.
- Educate patients and families on disease management, treatments, medications, and prevention.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary team members including therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and continuing education.
- Adhere to all regulatory and agency standards for documentation and patient care delivery.
Qualifications:
- Active Registered Nurse (RN) license in good standing within the state of practice.
- Minimum one year of recent home health experience; field experience preferred.
- Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments required.
- Strong clinical assessment, critical thinking, communication, and organizational skills.
- Reliable transportation with valid drivers license and auto insurance.
- Ability to work independently while managing a flexible field-based schedule.
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN) preferred.
- Experience managing 2532 points per week caseload.
- Knowledge of Medicare and home health compliance standards.
