Deliver compassionate, high-quality care as a skilled Home Health Registered Nurse, supporting patients with diverse needs in their own homes. This field-based contract offers rewarding opportunities to foster relationships and maximize patient outcomes while leveraging your clinical expertise in a supportive, autonomous environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Primary Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient status, promptly communicating changes to the healthcare team.
- Educate patients and families on disease processes, medication regimens, and preventive strategies.
- Document thoroughly and efficiently in Home Care Home Base (HCHB).
- Collaborate with interdisciplinary teams, including therapists, social workers, and home health aides.
- Attend case conferences, participate in staff meetings, and pursue continuing education opportunities.
- Uphold regulatory standards and best practices for in-home care and documentation.
Candidate Qualifications:
- Active and unencumbered RN license (state of practice).
- Minimum of one year of recent home health experience; field experience is preferred.
- Proficiency in Home Care Home Base (HCHB) and OASIS is required.
- Strong clinical assessment, critical thinking, and problem-solving skills.
- Reliable transportation, valid drivers license, and current auto insurance.
- Ability to work independently, manage caseloads, and excel in time management and communication.
- BSN and prior experience handling 2532 points/week are preferred.
- Familiarity with Medicare and home health compliance is a plus.
