Job Description

Deliver compassionate, high-quality care as a skilled Home Health Registered Nurse, supporting patients with diverse needs in their own homes. This field-based contract offers rewarding opportunities to foster relationships and maximize patient outcomes while leveraging your clinical expertise in a supportive, autonomous environment.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Primary Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor and evaluate patient status, promptly communicating changes to the healthcare team.
  • Educate patients and families on disease processes, medication regimens, and preventive strategies.
  • Document thoroughly and efficiently in Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary teams, including therapists, social workers, and home health aides.
  • Attend case conferences, participate in staff meetings, and pursue continuing education opportunities.
  • Uphold regulatory standards and best practices for in-home care and documentation.

Candidate Qualifications:

  • Active and unencumbered RN license (state of practice).
  • Minimum of one year of recent home health experience; field experience is preferred.
  • Proficiency in Home Care Home Base (HCHB) and OASIS is required.
  • Strong clinical assessment, critical thinking, and problem-solving skills.
  • Reliable transportation, valid drivers license, and current auto insurance.
  • Ability to work independently, manage caseloads, and excel in time management and communication.
  • BSN and prior experience handling 2532 points/week are preferred.
  • Familiarity with Medicare and home health compliance is a plus.
