Job Description

Bring your nursing expertise and independent drive to a rewarding contract opportunity specializing in home health care. As a Registered Nurse dedicated to delivering patient-centered care, you will play a crucial role in supporting individuals as they recover and manage chronic health conditions safely at home. This role emphasizes quality care, autonomy, and collaboration within an interdisciplinary team.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (typically 35 patient visits per day)

Responsibilities:

  • Conduct thorough OASIS assessments and tailor individualized care plans for each patient.
  • Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient conditions, respond promptly to changes, and communicate with physicians and care teams as needed.
  • Educate patients and their families on treatments, medications, disease processes, and preventive health.
  • Complete accurate electronic documentation using Home Care Home Base (HCHB).
  • Collaborate closely with therapists, medical social workers, and home health aides to ensure coordinated patient care.
  • Attend case conferences, staff meetings, and participate in ongoing professional development.
  • Uphold all regulatory and compliance standards in clinical practice and documentation.

Qualifications:

  • Active RN license (in good standing) in the state of practice.
  • At least 1 year of recent home health nursing experience.
  • Proficiency with HCHB and experience with OASIS assessments.
  • Strong clinical judgment, communication, and time management skills.
  • Reliable transportation, valid drivers license, and current auto insurance.
  • BSN and prior experience managing a weekly caseload of 2532 points preferred.
  • Familiarity with Medicare and home health compliance is a plus.
