Make a difference by delivering compassionate care directly to patients in their homes as an experienced Registered Nurse specializing in home health. This position offers a rewarding opportunity to provide high-quality, patient-centered nursing care, while collaborating with a multidisciplinary healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Role Highlights:
- Manage a field-based caseload averaging 2532 points per week (about 35 patient visits daily)
- Conduct detailed OASIS assessments and develop individualized care plans
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management
- Closely evaluate and document patient progress using Home Care Home Base (HCHB)
- Foster patient and family education on care processes, treatments, medication, and preventive strategies
- Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to coordinate comprehensive care
- Attend case conferences, staff meetings, and participate in continuing education
- Ensure all regulatory and agency standards for patient care and documentation are consistently met
Qualifications:
- Active RN license (in good standing) in the state of practice
- Minimum 1 year of recent home health experience (field experience highly valued)
- Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS assessment
- Strong clinical assessment, critical thinking, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
- BSN and prior experience managing 2532 points per week preferred
- Knowledge of Medicare and home health compliance standards a plus
