An exciting opportunity is available for an experienced and compassionate Registered Nurse with home health expertise. As a Home Health RN, you will deliver patient-centered, skilled nursing care in the comfort of your patients homessupporting their independence, quality of life, and recovery.

Schedule and Setting:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• 2532 points per week (typically 35 patient visits per day)
• Field-based (patient homes)
• Daily travel within an assigned territory; mileage reimbursement provided

Responsibilities:

• Complete comprehensive OASIS assessments and tailored care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient conditions, communicate changes promptly to healthcare teams, and adjust care plans accordingly.
• Educate patients and families on disease management, medication regimens, preventive strategies, and recovery.
• Maintain timely and thorough documentation using Home Care Home Base (HCHB).
• Collaborate with multidisciplinary team members (therapists, MSWs, aides) to ensure coordinated, holistic care.
• Participate in case conferences, staff meetings, and ongoing education.
• Uphold all regulatory, documentation, and agency standards.

Qualifications:

• Active, unencumbered RN license in the state of practice
• Minimum one year of recent field-based Home Health RN experience
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool required
• Strong clinical skills, critical thinking, and ability to work independently
• Reliable transportation, valid drivers license, and current auto insurance
• Excellent communication and organizational abilities
• BSN and experience managing 2532 points per week preferred
• Familiarity with Medicare and home health compliance is a plus

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
