As a Home Health Registered Nurse (RN), you'll be responsible for delivering skilled, patient-centered care to individuals recovering or managing chronic conditions outside the hospital setting. Enjoy the autonomy of an independent field-based schedule, balanced with strong support from an interdisciplinary healthcare team.
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM
What Youll Do:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing services such as wound care, IV therapy, medication administration, and management of chronic illnesses
- Monitor patient progress, recognize changes in condition, and communicate updates to the care team and physicians
- Provide education to patients and families on disease management, medication, and preventive care
- Maintain accurate, timely documentation using Home Care Home Base (HCHB) software
- Collaborate with therapists, medical social workers, and home health aides for coordinated care delivery
- Attend case conferences and staff meetings; participate in continuing education
What Were Looking For:
- Active RN license in good standing
- At least 1 year of recent home health field experience (required)
- Proficiency in Home Care Home Base (HCHB)
- OASIS assessment experience
- Strong clinical assessment, organization, and communication abilities
- Dependable transportation, valid drivers license, and current auto insurance
- Ability to manage a flexible, independent schedule
Preferred:
- BSN degree
- Previous management of a 2532 point caseload per week
- Knowledge of Medicare and home health compliance standards
