Job Description
Bring your expertise and compassion to end-of-life care as a Hospice Registered Nurse (RN), supporting patients and families with dignity and respect during their most vulnerable time. This position focuses on providing skilled nursing care within patients homes, assisted living, or nursing facilitiesa field-based role dedicated to individualized and supportive hospice services.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Occasional weekend/on-call rotation
- Approximately 45 patient visits per day
Key Responsibilities:
- Conduct comprehensive nursing assessments and develop individualized plans of care.
- Deliver symptom management, pain control, and direct patient care, supporting both comfort and quality of life.
- Administer medications and treatments according to established hospice protocols and physician orders.
- Monitor patient status, promptly communicate changes, and collaborate closely with medical directors and the care team.
- Provide education and guidance to patients, families, and caregivers regarding disease progression, symptom relief, and emotional support.
- Work collaboratively with an interdisciplinary team, including social workers, chaplains, and aides.
- Maintain timely, accurate documentation in Home Care Home Base (HCHB).
- Participate in ongoing quality improvement initiatives and team meetings.
- Ensure compliance with all hospice and regulatory standards.
Requirements:
- Active RN license (in good standing) in the state of practice
- Minimum of 1 year of recent hospice nursing experience
- Proficiency in Home Care Home Base (HCHB)
- Excellent assessment, communication, and organizational skills
- Strong background in pain management and family support
- Reliable vehicle, valid drivers license, and current auto insurance
- Compassionate, resilient, and autonomous approach to field-based care
Preferred:
- BSN degree
- Previous palliative or end-of-life care experience
- Familiarity with Medicare hospice regulations
