Step into a rewarding nursing role delivering care where it matters mostat home. As a Home Health Registered Nurse, you will leverage your clinical expertise and compassionate approach to ensure patients receive high-quality, personalized care in the comfort of their own residences.

Schedule:

• MondayFriday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Role Overview:

• Manage a caseload of 2532 points per week, averaging 35 patient visits each day in a field-based setting.
• Travel daily within your assigned territory (mileage reimbursement provided).
• Utilize Home Care Home Base (HCHB) for documentation and OASIS assessment tools.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and create tailored care plans for each patient.
• Deliver skilled nursing interventions: wound care, medication administration, IV therapy, chronic disease management, and more.
• Monitor patients progress, promptly reporting changes to care teams and physicians.
• Educate patients and families on treatment plans, disease management, and preventative care.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for cohesive patient care.
• Ensure thorough and timely clinical documentation and adhere to all regulatory standards.

Qualifications:

• Active RN license in good standing (state-specific).
• At least one year of recent field-based Home Health experience.
• Home Care Home Base (HCHB) proficiency and OASIS experience required.
• Strong assessment, communication, time management, and organizational skills.
• Reliable transportation, valid drivers license, and current auto insurance.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Prior experience managing a caseload of 2532 points per week.
• Familiarity with Medicare and home health compliance standards.

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
