Deliver high-quality, patient-centered care as a Home Health Registered Nurse in a field-based setting, providing vital support to individuals in the comfort of their own homes. This full-time, 13-week contract offers the opportunity to make a meaningful impact by supporting patients through education, comprehensive assessment, and skilled nursing interventions.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and create individualized care plans.
• Administer skilled nursing procedures such as medication management, wound care, IV therapy, and chronic disease monitoring.
• Monitor patient status, promptly report changes, and coordinate with physicians and the interdisciplinary care team.
• Educate patients and families on disease processes, medications, treatment plans, and preventative self-care.
• Maintain clear, timely documentation using Home Care Home Base (HCHB) electronic system.
• Work alongside physical and occupational therapists, medical social workers, and home health aides to ensure coordinated, comprehensive care.
• Attend case conferences, staff meetings, and pursue continuing education to uphold professional standards.
• Adhere to regulatory and agency practices for documentation and care delivery.

Qualifications:

• Current, active RN license in good standing.
• At least 1 year of recent Home Health RN experience, with field experience preferred.
• Demonstrated proficiency in HCHB and OASIS documentation.
• Reliable transportation, a valid drivers license, and up-to-date auto insurance.
• Strong clinical assessment, critical thinking, and organizational skills.
• Ability to work independently and manage a flexible, field-based workload.
• BSN and prior caseload management of 2532 points per week preferred.
• Familiarity with Medicare home health compliance a plus.
