Bring your nursing expertise to an enriching role dedicated to delivering high-quality care directly in patients homes. This full-time, 13-week opportunity is designed for an experienced and compassionate Registered Nurse with robust Home Health experience. You will independently manage a field-based caseload, making meaningful patient connections while supporting recovery and well-being in a comfortable, familiar environment.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based visits in patients homes
- Typical caseload: 2532 points per week (35 patient visits daily)
- Daily travel within your assigned area (mileage reimbursement provided)
Core Responsibilities:
- Perform thorough OASIS assessments and develop individualized care plans
- Administer skilled nursing interventions: wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient condition, promptly reporting changes and collaborating with care teams and physicians
- Educate patients and families on their diagnoses, treatment regimens, medications, and wellness strategies
- Ensure timely and accurate documentation using Home Care Home Base (HCHB)
- Work closely with physical and occupational therapists, medical social workers, and home health aides for coordinated care
- Participate in care conferences, staff meetings, and ongoing education as needed
- Maintain compliance with regulatory and agency standards
Qualifications:
- Active RN license in good standing in the state
- At least 1 year of recent Home Health field experience
- Required proficiency with Home Care Home Base (HCHB) and OASIS
- Excellent clinical assessment, communication, and time management skills
- Reliable transportation, valid drivers license, and proof of auto insurance
- BSN and experience managing similar caseloads preferred
- Familiarity with Medicare and home health regulations is advantageous
