A skilled and compassionate Registered Nurse is needed to deliver exceptional home health care to patients in a field-based setting. This role focuses on providing high-quality, patient-centered nursing services and ensuring a seamless continuum of care through collaboration with a multidisciplinary team. The contract is full-time and spans 13 weeks, with the following details:
Schedule & Hours:
- Monday to Friday, 8:00 AM 5:00 PM
- Field-based (patient homes), with daily travel and mileage reimbursement
- Caseload expectation: 2532 points per week (typically 35 patient visits per day)
- Utilizes the Home Care Home Base (HCHB) electronic documentation system and OASIS assessment tool
Key Responsibilities:
- Complete thorough OASIS assessments and develop individualized care plans
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, evaluate changes in condition, and communicate promptly with care teams and physicians
- Provide patient and family education on disease processes, medications, and preventative care
- Maintain accurate, compliant, and timely documentation via HCHB
- Coordinate with physical therapists, occupational therapists, medical social workers, and home health aides for integrated care
- Participate in case conferences, staff meetings, and ongoing professional development
- Adhere strictly to regulatory and agency standards
Qualifications:
- Active RN license in good standing for the state
- At least 1 year of recent home health (field) experience
- Proficiency with HCHB and OASIS is essential
- Strong assessment, organizational, and time management abilities
- Reliable transportation, current drivers license, and valid auto insurance
- BSN preferred; experience managing 25-32 point caseloads and knowledge of Medicare/home health compliance is a plus
