An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care to patients in their homes, ensuring continuity and patient-centered services. This role demands a self-motivated, organized professional who is comfortable working independently while upholding clinical excellence and clear communication.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and communicate promptly with physicians and the care team.
• Educate patients and families on disease processes, medications, treatments, and preventive care.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members including physical and occupational therapists, social workers, and aides.
• Participate in case conferences, staff meetings, and continuing education sessions.
• Adhere to all regulatory and agency standards.

Qualifications & Desired Experience:

• Active RN license in good standing in the state of practice.
• Minimum one year recent home health experience, field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment and critical thinking skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible, field-based schedule.
• Excellent communication, time management, and organizational skills.

Preferred:

• BSN preferred.
• Prior experience managing 2532 points per week.
• Knowledge of Medicare and home health compliance standards.

Schedule and Contract Details:

• Full-time, 13-week contract.
• Monday Friday, 8:00 AM 5:00 PM.
• Caseload expectation: 2532 points weekly (typically 35 patient visits per day).
• Daily travel to patient homes within assigned territory with mileage reimbursement.
