In this 13-week, full-time contract role, you will be responsible for providing skilled nursing services to individuals requiring medical support following illness, injury, or hospitalization.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based visits to patient homes
• Expectation: 2532 points per week (approximately 35 patient visits daily)

Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, promptly identifying and reporting health status changes to care teams and physicians
• Educate patients and families about disease processes, medications, and preventive care strategies
• Keep accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with other interdisciplinary team membersPT, OT, social workers, and aidesto coordinate optimal patient care
• Participate in regular case conferences, required staff meetings, and professional development activities
• Consistently adhere to regulatory requirements and agency policies for patient care and documentation

Qualifications:

• Active and unencumbered RN license in the state of practice
• At least 1 year of recent field-based Home Health RN experience
• Proficient in Home Care Home Base (HCHB) and OASIS assessment tools
• Reliable transportation, drivers license, and auto insurance
• Exceptional clinical assessment, organizational, and communication skills
• Ability to work independently and manage a flexible field-based schedule

Preferred:

• BSN degree
• Experience managing a caseload of 2532 points per week
• Knowledge of Medicare and home health compliance
