Job Description
Deliver quality patient care and make a meaningful difference as a Home Health Registered Nurse, working directly with individuals in their own residences. This contract opportunity offers autonomy, flexibility, and the chance to truly connect with patients, all while collaborating within a dedicated interdisciplinary team.
Schedule:
- Full-time, contract assignment (13 weeks)
- Monday to Friday, 8:00 AM 5:00 PM
- Caseload expectation: 2532 points per week (typically 35 patient visits daily)
Role Overview:
- Perform comprehensive OASIS assessments and create patient-centered care plans
- Provide skilled nursing interventions, including wound care, IV therapy, disease management, and medication administration
- Utilize Home Care Home Base (HCHB) for accurate electronic documentation
- Travel daily to patients homes within your assigned territory (mileage reimbursement provided)
- Educate patients and families on disease processes, medications, and health maintenance
- Coordinate care with physical and occupational therapists, medical social workers, and home health aides
- Participate in case conferences, staff meetings, and ongoing education to support best practices
- Ensure excellence in compliance, documentation, and communication with the healthcare team
Required Qualifications:
- Active RN license (in good standing)
- Minimum 1 year of recent home health experience (field/home visits preferred)
- Proficient in Home Care Home Base (HCHB) and OASIS documentation
- Reliable transportation, valid drivers license, and current auto insurance
- Strong clinical, communication, organizational, and critical thinking skills
- Ability to work independently and adapt to a field-based role
Preferred:
- BSN degree
- Experience managing 2532 points per week
- Knowledge of Medicare and home health compliance
