Seeking an experienced and dedicated Registered Nurse to deliver quality care in a home health setting. This contract opportunity allows you to make a meaningful impact on patients well-being by providing skilled nursing services within their homes. You will play a key role in coordinating and implementing evidence-based care plans designed to improve patient outcomes and provide vital education to patients and families.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Responsibilities:

• Conduct comprehensive patient assessments using the OASIS tool and develop individualized home health care plans
• Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease oversight
• Monitor patient progress, identify changes in condition, and communicate promptly with physicians and interdisciplinary team members
• Educate patients and families regarding disease processes, treatment plans, medications, and preventive strategies
• Complete accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with physical, occupational therapy, medical social workers, and home health aides to ensure coordinated care
• Participate in case conferences and continuing education activities
• Maintain compliance with agency, local, and federal guidelines for home health care

Requirements:

• Active RN License (state of practice, in good standing)
• Minimum 1 year of recent home health experience (field work strongly preferred)
• Proficiency with OASIS and Home Care Home Base (HCHB)
• Strong clinical assessment, critical thinking, organization, and time management skills
• Reliable transportation with valid drivers license and auto insurance
• Ability to work independently in the field with a flexible schedule

Preferred:

• BSN degree
• Experience managing 2532 points per week
• Familiarity with Medicare and home health compliance

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
