An opportunity is available for an experienced Home Health Registered Nurse seeking a dynamic and patient-centered role. This contract assignment offers the chance to deliver skilled nursing care in a home-based setting, serving individuals who value compassion, professionalism, and clinical excellence. Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based (patient homes)
• Caseload: 2532 points per week (about 35 patient visits daily)
• Daily travel within assigned territory (mileage reimbursement available)

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Assess and monitor patient progress, promptly reporting changes to physicians and care teams.
• Educate patients and families on health conditions, care plans, and preventive measures.
• Maintain detailed documentation in Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to coordinate care.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with regulatory and agency standards.

Requirements:

• Active RN license (in good standing)
• At least one year of recent field-based Home Health experience
• Proficiency with Home Care Home Base (HCHB) and OASIS
• Excellent assessment, critical thinking, and communication skills
• Valid drivers license, reliable transportation, and auto insurance
• Ability to work independently in a flexible field schedule

Preferred:

• BSN
• Experience managing a 2532 point weekly caseload
• Familiarity with Medicare and home health compliance

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
