An exciting opportunity is available for an experienced and compassionate Registered Nurse to deliver quality care in a home health setting. This full-time, 13-week contract role focuses on providing patient-centered nursing for individuals in their homes, supporting continuity of care and positive health outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits each day)

Key Responsibilities:

• Complete comprehensive OASIS assessments and design individualized plans of care
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor and evaluate patient progress and report changes to the healthcare team
• Provide thorough patient and family education on treatment plans, medications, and disease processes
• Document care promptly and accurately in Home Care Home Base (HCHB)
• Collaborate with physical and occupational therapists, social workers, and aides to ensure coordinated, high-quality service
• Attend case conferences, staff meetings, and maintain up-to-date professional knowledge
• Ensure compliance with all regulatory, documentation, and agency standards

Required Qualifications:

• Active and unencumbered RN license
• Minimum 1 year recent experience in home health, ideally with field-based responsibilities
• Familiarity and proficiency with HCHB and OASIS
• Excellent clinical assessment, critical thinking, and organization skills
• Reliable transportation with valid drivers license and auto insurance
• Ability to work independently with strong communication skills

Preferred Qualifications:

• BSN degree
• Experience with caseload of 2532 points per week
• Understanding of Medicare guidelines and home health compliance

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
