Job Description
Deliver skilled, compassionate nursing care in a home health setting as a Registered Nurse supporting patients recovery in the comfort of their own residences. This role offers a dynamic field-based schedule and the opportunity to build meaningful patient relationships while collaborating closely with an interdisciplinary team to ensure the best possible outcomes.
Schedule & Setting:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Approximately 2532 points per week (typically 35 patient visits per day)
- Daily travel within assigned territory (mileage reimbursement provided)
- Documentation via Home Care Home Base (HCHB); OASIS assessments required
Key Responsibilities:
- Complete thorough OASIS admissions, assessments, and develop individualized care plans for each patient.
- Deliver skilled nursing services, including medication management, wound care, IV therapy, and chronic disease management.
- Monitor clinical progress, identify changes in condition, and collaborate with physicians and team members to adjust plans of care as needed.
- Educate patients and families on diagnoses, medications, treatment regimens, and preventative health.
- Maintain thorough, accurate documentation in HCHB in compliance with regulatory requirements.
- Work collaboratively with therapists, social workers, and aides to ensure continuity and quality of care.
- Participate in staff meetings, case conferences, and ongoing educational opportunities.
Qualifications:
- Valid RN license (good standing) in the state of practice.
- At least 1 year of recent home health nursing experience, field-based work preferred.
- Proficient in Home Care Home Base (HCHB) and OASIS.
- Excellent critical thinking, time management, and communication skills.
- Reliable transportation with valid drivers license and auto insurance.
- BSN preferred. Familiarity with Medicare compliance is a plus.
