Delivering skilled and compassionate care directly to patients in their homes, this vital nursing role supports individuals health and independence through high-quality, patient-centered services. As a Home Health Registered Nurse (RN) on a 13-week contract, your expertise will help ensure that patients receive seamless, individualized care with a focus on positive outcomes and superior clinical standards.
Schedule & Work Expectations:
- Monday through Friday, 8:00 AM 5:00 PM
- Field-based (home visits) with daily travel within an assigned territory
- Caseload of 2532 points per week, typically 35 patient visits daily
Role Responsibilities:
- Conduct comprehensive OASIS assessments and create care plans tailored to each patients needs
- Deliver skilled nursing interventions, such as wound care, IV therapy, medication management, and chronic condition oversight
- Monitor patient progress, identify changes in health status, and communicate promptly with care teams and physicians
- Educate patients and families on care plans, medications, disease management, and preventive health practices
- Maintain accurate, timely documentation using Home Care Home Base (HCHB)
- Work collaboratively with interdisciplinary team members, including therapists, social workers, and aides
- Participate in case conferences and maintain compliance with all regulatory standards
Qualifications:
- Active RN license in good standing
- At least 1 year of recent field experience in home health nursing
- Proficiency with Home Care Home Base (HCHB) electronic documentation and OASIS assessments
- Strong clinical judgment, organization, and communication abilities
- Reliable transportation, valid drivers license, and auto insurance required
- BSN and experience managing a caseload of 2532 points/week preferred
- Solid knowledge of Medicare and home health compliance is a plus
