Job Description

A skilled Home Health Registered Nurse (RN) position is available to deliver patient-centered nursing care in a home setting. This full-time role involves managing a caseload of 2532 points weekly, equating to 35 patient visits per day, within a structured Monday through Friday, 8:00 AM to 5:00 PM schedule. The role requires daily travel within an assigned territory, with mileage reimbursement provided.

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop tailored care plans.
  • Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify any changes in condition, and communicate findings to the care team and physicians.
  • Educate patients and families on disease management, treatment plans, medications, and preventative measures.
  • Maintain accurate documentation using Home Care Home Base (HCHB).
  • Collaborate with therapy professionals, social workers, and aides to ensure cohesive care delivery.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Comply with all regulatory and agency requirements.

Qualifications:

  • Active RN license in good standing within the state of practice.
  • Minimum of one year of recent home health nursing experience with field experience preferred.
  • Proficiency in Home Care Home Base (HCHB) and OASIS assessments required.
  • Strong clinical assessment, critical thinking, communication, organizational, and time management skills.
  • Valid drivers license, reliable transportation, and auto insurance.
  • Ability to work independently and manage a flexible schedule.

Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Previous experience managing 2532 points per week.
  • Familiarity with Medicare and home health compliance standards.
