Deliver compassionate, high-quality nursing care directly to patients homes as an experienced Home Health Registered Nurse. Utilize your clinical expertise to perform thorough assessments, develop personalized care plans, and foster positive patient outcomes. You will operate autonomously in a field-based environment while collaborating with an interdisciplinary care team to ensure the highest standard of service and documentation.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Field-based (patient homes) with daily travel required (mileage reimbursement provided)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and tailor individualized care plans.
• Deliver skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and coordinate with physicians and the broader healthcare team.
• Educate patients and families about disease management, medications, and preventive care strategies.
• Ensure accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, medical social workers, and aides for coordinated care delivery.
• Participate in case conferences, team meetings, and ongoing education.
• Comply with all agency and regulatory standards for documentation and care.

Qualifications:

• Active RN License in good standing.
• Minimum 1 year of recent field-based Home Health experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments required.
• Excellent assessment, communication, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible schedule.

Preferred:

• BSN degree.
• Prior experience with a 2532 point caseload.
• Familiarity with Medicare regulations and compliance.
