Experienced and compassionate Registered Nurses with home health expertise are invited to play a vital role in delivering patient-centered care in a rewarding field-based setting. This full-time, 13-week contract position focuses on providing skilled nursing services to patients in their homes, ensuring continuity of care and improved patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and create individualized care plans for each patient.
- Deliver skilled nursing interventions, such as wound care, medication administration, IV therapy, and management of chronic health conditions.
- Monitor patient progress, identify health status changes, and communicate promptly with physicians and the care team.
- Educate patients and families about disease processes, medications, treatment plans, and preventive strategies.
- Ensure accurate, timely documentation through Home Care Home Base (HCHB).
- Coordinate care with physical therapists, occupational therapists, medical social workers, and home health aides.
- Participate in case conferences, staff meetings, and relevant continuing education.
- Adhere to all regulatory and agency documentation and care delivery standards.
Qualifications:
- Active, unencumbered RN license in the state of practice.
- At least one year of recent home health experience (field experience highly preferred).
- Proficiency with Home Care Home Base (HCHB) electronic documentation.
- Demonstrated OASIS assessment experience.
- Excellent assessment, communication, and organizational skills.
- Ability to work independently, effectively manage a field-based caseload (2532 points/week), and maintain a flexible schedule.
- Reliable transport, valid drivers license, and auto insurance.
- BSN and prior experience handling a similar caseload preferred.
- Knowledge of Medicare and home health compliance is a plus.
