Experienced and compassionate Registered Nurses with home health expertise are invited to play a vital role in delivering patient-centered care in a rewarding field-based setting. This full-time, 13-week contract position focuses on providing skilled nursing services to patients in their homes, ensuring continuity of care and improved patient outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and create individualized care plans for each patient.
• Deliver skilled nursing interventions, such as wound care, medication administration, IV therapy, and management of chronic health conditions.
• Monitor patient progress, identify health status changes, and communicate promptly with physicians and the care team.
• Educate patients and families about disease processes, medications, treatment plans, and preventive strategies.
• Ensure accurate, timely documentation through Home Care Home Base (HCHB).
• Coordinate care with physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and relevant continuing education.
• Adhere to all regulatory and agency documentation and care delivery standards.

Qualifications:

• Active, unencumbered RN license in the state of practice.
• At least one year of recent home health experience (field experience highly preferred).
• Proficiency with Home Care Home Base (HCHB) electronic documentation.
• Demonstrated OASIS assessment experience.
• Excellent assessment, communication, and organizational skills.
• Ability to work independently, effectively manage a field-based caseload (2532 points/week), and maintain a flexible schedule.
• Reliable transport, valid drivers license, and auto insurance.
• BSN and prior experience handling a similar caseload preferred.
• Knowledge of Medicare and home health compliance is a plus.
