Provide expert nursing care to patients in their homes as a Home Health Registered Nurse (RN), delivering personalized and compassionate care that supports patient health and independence. This full-time, 13-week contract opportunity offers a professional environment to apply your nursing skills within a field-based setting.

This position is based in the Dacula, GA area, a community known for its balance of suburban comfort and access to nearby metropolitan amenities. You'll enjoy a region rich in parks, local dining, and convenient transportation options, making daily travel to patient homes both manageable and rewarding.

Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions including wound care, medication management, IV therapy, and chronic disease oversight.
• Monitor patient progress, recognize clinical changes, and communicate effectively with healthcare teams and physicians.
• Provide education to patients and families on treatment plans, medication use, disease management, and preventive care.
• Maintain precise and prompt electronic documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members, including therapists and social workers, to ensure comprehensive care.
• Participate in case conferences, ongoing training, and comply with all regulatory standards.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum of 1 year recent home health nursing experience (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools is required.
• Strong clinical judgment, organization, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance necessary.
• Ability to work independently managing a flexible, field-based schedule.

Preferred:

• Bachelor of Science in Nursing (BSN) preferred.
• Experience managing a caseload of 2532 points weekly.
• Knowledge of Medicare and home health compliance standards.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• Typically 35 patient visits per day
