Deliver skilled nursing care with autonomy and compassion by supporting patients as a Home Health Registered Nurse in a respected community setting. This contract position allows you to have a direct, positive impact on patient outcomes while utilizing your leadership and clinical assessment abilities.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Role Highlights:

• 13-week contract, full-time
• Daily travel within assigned territory (mileage reimbursement provided)
• Caseload: 2532 points per week (approximately 35 patient visits per day)
• Field-based (patient homes)
• Electronic charting with Home Care Home Base (HCHB)
• Comprehensive patient assessments using OASIS

Primary Duties:

• Perform thorough OASIS assessments and create tailored plans of care
• Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, swiftly communicate clinical changes, and coordinate care with interprofessional teams
• Educate patients and families about their treatment, disease management, and medication
• Maintain up-to-date, compliant documentation within HCHB
• Participate in interdisciplinary meetings, continuous learning, and maintain regulatory standards

Required Qualifications:

• Valid, active RN license (state of practice)
• Minimum of 1 year recent home health experience (field visits)
• Proficiency with HCHB and OASIS documentation
• Solid clinical judgment and assessment skills
• Independent, detail-oriented, and flexible with strong communication abilities
• Valid drivers license, reliable transportation, and auto insurance

Preferred:

• BSN
• Experience handling 2532 points/week
• Familiarity with Medicare and home health compliance

$1,839 - $1,946/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
