An opportunity is available for an experienced and compassionate Registered Nurse to deliver high-quality care in a home health setting. This contract position focuses on supporting patients and families throughout their healthcare journey, emphasizing quality, compliance, and individual attention. The role is suited for nurses who excel at working independently and thrive in a dynamic, field-based environment.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct OASIS assessments and create patient-centered care plans
- Deliver skilled nursing care in patients homes, including medication administration, wound care, IV therapy, and management of chronic conditions
- Monitor patient progress, identify changes in status, and coordinate care with interdisciplinary teams
- Educate patients and their families on treatment plans, medications, and disease management
- Ensure timely and accurate documentation in Home Care Home Base (HCHB)
- Collaborate with healthcare professionals such as therapists and social workers for comprehensive support
- Maintain regulatory compliance and participate in team meetings and continuing education
Qualifications:
- Current, active RN license (in good standing)
- At least 1 year of recent home health field experience
- Proficiency with the Home Care Home Base (HCHB) electronic documentation system
- Demonstrated OASIS assessment experience
- Robust clinical assessment skills and critical thinking abilities
- Reliable transportation, valid drivers license, and auto insurance
- Competency in working independently and managing schedules
- Strong communication, organization, and time management skills
- BSN and knowledge of Medicare/home health standards preferred
