An opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) to deliver quality skilled care in a home-based setting. This full-time, 13-week contract position is ideal for nursing professionals who excel working independently and have a passion for patient-centered home health services.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Position Details:
- Field-based, providing care at patient homes within an assigned territory
- Daily travel required; mileage reimbursement provided
- Electronic documentation using Home Care Home Base (HCHB)
- Caseload expectation: 2532 points per week (typically 35 patient visits daily)
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Provide skilled nursing care: wound care, medication administration, IV therapy, chronic disease management
- Monitor patient progress, identify changes in condition, and communicate with care teams and physicians
- Educate patients and families regarding disease processes, treatments, medications, and preventive care
- Complete accurate and timely documentation in HCHB
- Collaborate with interdisciplinary team members to ensure coordinated, compliant care
- Participate in case conferences, staff meetings, and professional development as needed
- Adhere to regulatory and agency care standards
Desired Qualifications:
- Active RN license (in good standing)
- Minimum of one year recent Home Health experience (field-based preferred)
- Proficiency with HCHB and OASIS required
- Strong clinical assessment, time management, and communication abilities
- Reliable transportation, valid drivers license, and current auto insurance required
- BSN and experience managing 2532 points weekly preferred
- Knowledge of Medicare and home health compliance is a plus
