Deliver compassionate, high-quality care as a Home Health Registered Nurse with a dedicated team supporting patients in the comfort of their own homes. This rewarding contract opportunity empowers you to provide personalized nursing care, promote positive outcomes, and build meaningful relationships with patients and their families.
Schedule/Hours:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload expectation: 2532 points per week (typically 35 patient visits per day)
- Field-based role with daily travel in assigned territory
Qualifications:
- Active, unencumbered RN license in the state of practice
- Minimum 1 year of recent Home Health experience (field experience strongly preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Exceptional clinical assessment, communication, and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible field schedule
- BSN and knowledge of Medicare/home health compliance standards preferred
- Experience managing 2532 points per week strongly desired
Primary Responsibilities:
- Conduct comprehensive OASIS assessments and create individualized plans of care
- Deliver skilled nursing services (wound care, IV therapy, medication administration, chronic disease management)
- Monitor patient progress, document care in HCHB, and collaborate with interdisciplinary teams
- Provide education to patients and families on disease processes, treatment plans, and preventive care
- Participate in case conferences and staff meetings
- Ensure adherence to regulatory and agency protocols
