Job Description

Make a real difference providing high-quality care as a Home Health Registered Nurse (RN) supporting patients in their own homes. This contract opportunity focuses on skilled nursing services with an emphasis on patient-centered care and clinical excellence. Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Caseload: 2532 points per week (approx. 35 patient visits daily)

Key Responsibilities:

  • Conduct OASIS assessments and develop patient-specific care plans
  • Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress, communicate changes, and collaborate closely with healthcare teams and physicians
  • Educate patients and families regarding care plans, disease management, medications, and preventive strategies
  • Maintain documentation in Home Care Home Base (HCHB)
  • Participate in interdisciplinary meetings, staff conferences, and ongoing training
  • Comply with all professional, regulatory, and agency standards

Qualifications:

  • Active RN license (in good standing) in state of practice
  • Minimum 1 year of recent Home Health field experience
  • Proficiency with HCHB and OASIS required
  • Strong assessment and critical-thinking abilities
  • Reliable transportation, valid drivers license, and auto insurance
  • Excellent organizational, communication, and time-management skills
  • BSN and experience with Medicare/home health compliance preferred

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
