Job Description

An exciting opportunity awaits an experienced and compassionate Registered Nurse with a commitment to home health care. This full-time, 13-week contract position involves providing high-quality nursing services directly in patients homes, ensuring comprehensive and patient-focused support throughout the care continuum.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Complete comprehensive OASIS assessments and develop personalized care plans
  • Deliver skilled nursing interventions: wound care, IV therapy, medication management, and chronic disease monitoring
  • Monitor patient progress, identify changes in condition, and communicate quickly with the interdisciplinary team and physicians
  • Educate patients and families on treatment plans, medications, self-care, and preventive strategies
  • Perform accurate, timely documentation within the Home Care Home Base (HCHB) electronic system
  • Collaborate with physical and occupational therapists, social workers, and home health aides to ensure coordinated care
  • Attend case conferences, staff meetings, and ongoing education sessions as needed
  • Maintain all standards of documentation and care in accordance with current regulations and agency policies

Qualifications:

  • Active RN license (state of practice, in good standing)
  • At least 1 year of recent experience in Home Health (field work preferred)
  • Strong proficiency with Home Care Home Base (HCHB) and OASIS assessments
  • Valid drivers license, reliable vehicle, and current auto insurance
  • Demonstrated ability to work independently and manage a mobile schedule
  • Excellent communication, critical thinking, and organizational skills
  • BSN and familiarity with Medicare and home health compliance standards preferred
