Deliver patient-centered care as a Home Health Registered Nurse, providing skilled nursing services to individuals in their homes. This contract opportunity offers direct patient interaction, a supportive environment, and efficient tools for delivering clinical care. Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Perform OASIS assessments and develop individualized care plans for home health patients.
• Deliver skilled nursing interventions, such as medication administration, wound care, IV therapy, and chronic disease management.
• Monitor patient progress, recognize shifting conditions, and communicate findings to the healthcare team and physicians.
• Educate patients and families regarding diagnoses, medications, treatment regimens, and preventive strategies.
• Complete timely documentation using Home Care Home Base (HCHB).
• Coordinate care with therapists, social workers, and home health aides.
• Actively participate in case conferences and staff meetings to ensure continuity of care.
• Uphold all required regulatory, documentation, and patient care standards.

Required Qualifications:

• Active RN license (state-specific and in good standing)
• At least one year of recent home health nursing experience (field preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid drivers license, and up-to-date auto insurance
• Strong clinical skills, organizational ability, and independent work ethic

Preferred:

• BSN degree
• Previous experience managing 2532 points per week
• Knowledge of Medicare and home health compliance
