As a Home Health Registered Nurse (RN), you'll use your expertise to assess, plan, and oversee care for individuals recovering or living with chronic conditions in the comfort of their homes. This full-time, 13-week contract role offers you the autonomy to work independently, ensuring best outcomes through compassionate, high-quality nursing care.

Schedule and Assignment Details:

• Monday through Friday, 8:00 AM 5:00 PM
• Field-based position (visiting patient homes)
• Manage 2532 points per week, typically 35 visits daily
• Daily travel within your assigned territory (mileage reimbursement provided)
• Utilization of Home Care Home Base (HCHB) for documentation and OASIS for assessment

Key Responsibilities:

• Conduct thorough OASIS assessments and establish individualized care plans
• Administer interventions such as wound care, IV therapy, medication management, and chronic disease support
• Monitor progress, respond to patient needs and changes in condition, and provide effective communication with interdisciplinary care teams and physicians
• Deliver education to patients and caregivers on disease management, medications, treatments, and preventive strategies
• Maintain accurate records in HCHB, complying with all regulatory and agency standards
• Engage in collaborative meetings, ongoing education, and professional development

Qualifications:

• Active RN License (in good standing)
• At least 1 year of recent, field-based home health RN experience
• Proficient in HCHB and OASIS
• Excellent assessment, organizational, and communication abilities
• Reliable transportation, valid drivers license, and current auto insurance
• BSN and knowledge of Medicare home health compliance standards preferred

$2,011 - $2,127/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
