Deliver high-quality nursing care directly to patients in their homes as part of a dynamic, supportive healthcare team. This full-time, 13-week contract position focuses on delivering compassionate and skilled nursing services, supporting recovery and independence for patients throughout the community.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and formulate individualized care plans
• Provide clinical interventions such as wound care, IV therapy, and chronic disease management in the home setting
• Administer medications, monitor patient progress, and document all care accurately using Home Care Home Base (HCHB)
• Communicate observations and changes in patient condition promptly with physicians and interdisciplinary team members
• Deliver patient and caregiver education on managing illnesses, medications, and overall health
• Collaborate with physical therapists, occupational therapists, social workers, and aides to ensure seamless care coordination
• Participate in team meetings, case conferences, and ongoing education to maintain best practices and compliance

Requirements:

• Active RN license in the state (good standing)
• Minimum 1 year of current home health nursing experience (field experience preferred)
• Strong proficiency in HCHB and OASIS documentation
• Skilled in clinical assessment, organization, and independent decision-making
• Reliable transportation, valid drivers license, and proof of auto insurance
• Demonstrated ability to manage a flexible, field-based schedule with excellent communication skills

Preferred:

• BSN degree
• Experience with caseloads of 2532 points per week
• Familiarity with Medicare and home health regulations
