As a Home Health Registered Nurse (RN) on a full-time, 13-week contract, you will deliver skilled, patient-centered care in the comfort of your patients homes. This field-based position is ideal for a self-motivated clinician who excels working independently while upholding outstanding clinical standards and communication.

Schedule & Structure:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Expect a caseload of 2532 points per week (typically 35 patient visits daily)
• Travel throughout an assigned territory (mileage reimbursement provided)
• Utilize Home Care Home Base (HCHB) for documentation; OASIS assessment tool

Key Responsibilities:

• Conduct OASIS assessments; develop and implement individualized care plans
• Deliver skilled nursing care (wound care, IV therapy, chronic disease management, etc.)
• Monitor patient status, report changes, and adjust care plans as needed
• Educate patients and families on health conditions, medications, and prevention
• Maintain accurate, timely charting in HCHB
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides
• Participate in multidisciplinary meetings and ongoing education
• Ensure compliance with all clinical, regulatory, and documentation standards

Qualifications:

• Active RN license in good standing (state of practice)
• At least one year of recent home health nursing experience (field experience highly preferred)
• Proficient in Home Care Home Base (HCHB)
• Demonstrated experience with OASIS
• Reliable transportation, valid drivers license, and proof of auto insurance
• Strong clinical, organizational, and communication skills; ability to work autonomously
• BSN preferred
• Experience handling 2532 points per week and knowledge of Medicare compliance a plus

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
