An exciting opportunity is available for an experienced and compassionate Registered Nurse to deliver high-quality care in a home health setting. This contract role places you at the forefront of patient-centered care, performing skilled nursing services to individuals in the comfort of their homes while supporting them on their journey to improved health.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities include:
- Completing comprehensive OASIS assessments and customizing care plans for patients.
- Providing skilled nursing interventions such as wound care, medication administration, IV therapy, and education for chronic disease management.
- Monitoring patient progress, identifying changes in condition, and communicating updates with physicians and care teams.
- Educating patients and families regarding diagnoses, treatment regimens, medication instructions, and preventive care measures.
- Documenting visits and care provided using Home Care Home Base (HCHB).
- Coordinating with physical therapists, occupational therapists, social workers, and home health aides to ensure continuity of care.
- Participating in case conferences, staff meetings, and ongoing education.
- Adhering to regulatory and agency standards for documentation and patient care delivery.
- Traveling daily to patient homes within your assigned area, with mileage reimbursement provided.
Qualifications:
- Active RN License (in good standing) within the state of practice.
- At least 1 year of recent home health experience (field work preferred).
- Experience with OASIS and proficiency in Home Care Home Base (HCHB) required.
- Strong clinical assessment skills and ability to work independently.
- Reliable transportation, valid drivers license, and auto insurance.
- Strong organization, time management, and communication skills.
- BSN preferred.
- Familiarity with Medicare and home health compliance standards is a plus.
