Job Description
Deliver skilled nursing care within a dynamic home health environment, ensuring patients receive optimal support in their own homes. This full-time, 13-week contract role calls for an experienced and compassionate Registered Nurse who thrives on autonomy and values high standards in both clinical care and communication.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Field-based: travel within assigned territory (mileage reimbursement provided)
Core Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
- Evaluate patient condition, progress, and response to care; communicate changes promptly with the interdisciplinary care team and physicians.
- Educate patients and families on disease processes, medication regimens, and preventive health practices.
- Accurately document all patient information in Home Care Home Base (HCHB) electronic records.
- Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated, holistic care.
- Participate in case conferences, staff meetings, and occasional continuing education.
- Ensure compliance with regulatory and agency care standards.
Qualifications:
- Active and unrestricted RN license in the state of practice
- Minimum of one year of recent field-based Home Health RN experience
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Strong clinical, assessment, and communication skills
- Reliable personal transport, valid drivers license, and auto insurance
- Ability to work independently and manage a field schedule
- BSN and Medicare knowledge preferred
