Key Qualifications:

• Active and unrestricted RN license
• Recent clinical experience in home health settings
• Comprehensive understanding of OASIS documentation
• Ability to assess, plan, implement, and evaluate patient care independently
• Strong communication skills for effective collaboration with interdisciplinary teams
• Proficiency with electronic medical records and mobile documentation platforms
• Current CPR certification and valid drivers license

Responsibilities:

• Deliver high-quality nursing care to homebound patients, following care plans and physicians orders
• Accurately complete clinical documentation and home health assessments
• Collaborate with other healthcare professionals, patient families, and caregivers to achieve optimal outcomes
• Administer medications, monitor vital signs, and provide education on disease management and safety
• Ensure compliance with regulatory standards and maintain confidentiality of patient information
• Participate in care team meetings and quality improvement initiatives

$1,573 - $1,664/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
