Deliver patient-centered care as an experienced and compassionate Home Health Registered Nurse (RN) dedicated to serving individuals in their homes. This full-time, 13-week contract position offers you the opportunity to provide comprehensive clinical support while fostering independence and improving quality of life for each patient.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, medication management, IV therapy, and chronic disease support.
• Monitor patient progress, recognize changes in condition, and collaborate with interdisciplinary teams and physicians.
• Educate patients and family members about conditions, care processes, medications, and preventative strategies.
• Accurately document care using Home Care Home Base (HCHB) electronic system.
• Participate in case conferences and required staff meetings, maintaining regulatory and agency standards.

Qualifications:

• Active, unencumbered RN license in the state of practice.
• At least 1 year of recent home health nursing experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation is required.
• Exceptional assessment, clinical judgment, and communication skills.
• Ability to work independently, efficiently manage schedule, and travel daily within assigned territory (mileage reimbursement provided).
• Reliable transportation with valid drivers license and automobile insurance.
• BSN and prior experience managing 2532 points per week preferred.
• Familiarity with Medicare and home health compliance standards is a plus.

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
