An exciting opportunity awaits for an experienced and compassionate Registered Nurse with a background in home health. This full-time, 13-week contract role is ideal for clinicians who thrive working autonomously while making a meaningful difference in the lives of patients. As a Home Health RN, you'll play a key role in delivering high-quality care directly in patient homes, ensuring continuity and promoting positive outcomes through thorough assessment and management.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress, recognize changes in condition, and communicate findings to care teams and physicians.
• Educate patients and families on disease management, treatment plans, and medications.
• Accurately document all care activities in Home Care Home Base (HCHB).
• Coordinate with therapists, social workers, and home health aides to optimize patient outcomes.
• Attend case conferences and staff meetings, and participate in ongoing education.
• Ensure compliance with regulatory guidelines and quality standards.

Qualifications:

• Active RN license in good standing (state of practice).
• At least one year of recent field-based home health experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Excellent clinical assessment, communication, and organizational skills.
• Ability to manage a caseload of 2532 points per week (35 visits daily).
• Reliable transportation, valid drivers license, and auto insurance.
• BSN and Medicare compliance knowledge preferred.
