Provide expert, patient-centered care as a Home Health Registered Nurse delivering compassionate services to individuals in their residences. This contract position offers the opportunity to make a direct impact, supporting clients through the full spectrum of home-based nursing care. Youll be responsible for developing individualized plans, monitoring progress, ensuring regulatory compliance, and fostering communication with both patients and interdisciplinary healthcare teams.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM (Full-time, 13-week contract)
- Daily travel required within assigned territory for patient visits (mileage reimbursement provided)
- Caseload expectation: 2532 points per week (typically 35 visits daily)
- Field-based position using Home Care Home Base (HCHB) and OASIS assessments
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop tailored care plans.
- Deliver skilled home health nursing including IV therapy, wound care, medication administration, and chronic condition management.
- Monitor changes in patient conditions, communicating updates to the care team and physicians.
- Educate patients and families about diagnoses, care plans, and preventive strategies.
- Maintain accurate, timely documentation within Home Care Home Base (HCHB).
- Coordinate care with therapists, social workers, and home health aides.
- Participate in case conferences, meetings, and ongoing training.
- Uphold all clinical, regulatory, and agency documentation standards.
Qualifications:
- Valid RN license in good standing for the state of practice.
- At least one year of recent home health (field) RN experience.
- Required: Experience with Home Care Home Base (HCHB) and OASIS.
- Excellent clinical assessment, time management, and communication skills.
- Reliable transportation, valid drivers license, and current auto insurance.
- BSN and knowledge of Medicare/home health compliance are preferred.
