Job Description
Hospice nursing requires a deep commitment to compassionate end-of-life care. This contract opportunity offers you the chance to support patients and families, utilizing your clinical expertise and empathy in a meaningful way. As a field-based Hospice RN, you'll have the flexibility and autonomy to provide exceptional hospice services across a range of home settings.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
- Approximately 45 patient visits per day
Qualifications:
- Active RN license (in good standing)
- At least 1 year of recent hospice experience required
- Proficiency in Home Care Home Base (HCHB) electronic documentation required
- Excellent assessment, clinical, and communication skills
- Strong knowledge of symptom management and end-of-life care practices
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work independently and manage daily travel within the service area
- Compassionate, adaptable, and emotionally resilient
Preferred Experience:
- Bachelor of Science in Nursing (BSN)
- Prior work in palliative or similar end-of-life care environments
- Familiarity with Medicare hospice regulations and documentation standards
Key Responsibilities:
- Conduct holistic assessments and develop personalized hospice care plans
- Deliver skilled nursing care for comfort and symptom management
- Administer medications and interventions as ordered
- Educate families about disease trajectory, comfort techniques, and medication administration
- Collaborate with interdisciplinary team members to support all aspects of the patients journey
- Maintain accurate, timely documentation in HCHB
- Participate in quality improvement activities and team conferences
