Make a difference every day by providing expert care directly in patients homes as a Home Health Registered Nurse (RN). This is a full-time role with a 13-week contract, offering the chance to lead patient-centered care and promote positive health outcomes in a flexible, field-based setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments to develop and manage individualized care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress, identify changes in condition, and communicate promptly with care teams and physicians.
• Provide education to patients and their families regarding disease processes and preventive care.
• Accurately document all care in Home Care Home Base (HCHB).
• Collaborate with interdisciplinary healthcare providers such as physical therapists, occupational therapists, and social workers.
• Participate in case conferences, staff meetings, and continuing education sessions.
• Adhere consistently to regulatory and agency documentation and patient care standards.

Qualifications:

• Active RN license (in good standing) in the state of practice.
• At least 1 year of recent home health experience (field experience preferred).
• Proficiency with OASIS and electronic documentation in Home Care Home Base (HCHB).
• Reliable transportation, valid drivers license, and auto insurance.
• Exceptional clinical assessment, organizational, and communication skills.
• Ability to manage independent, flexible field-based schedules.

Preferred Attributes:

• BSN degree.
• Prior experience with caseloads of 2532 points per week.
• Understanding of Medicare and home health compliance guidelines.
