An excellent opportunity awaits an experienced and compassionate Registered Nurse to provide skilled nursing care to patients in their homes. This contract role is ideal for a self-motivated RN who thrives working independently and is committed to clinical excellence and patient-centered care. The successful candidate will deliver comprehensive assessments, create effective care plans, and act as a valuable resource for patients and families throughout their health journey.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Field-based, daily travel within assigned territory

Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, recognize changes in status, and promptly update care teams and physicians
• Educate patients and families on disease processes, medications, and preventive care strategies
• Maintain timely, accurate documentation using Home Care Home Base (HCHB)
• Collaborate with multidisciplinary teams, including therapists and social workers, to ensure coordinated home health services
• Participate in case conferences, staff meetings, and ongoing education

Qualifications:

• Active RN license (in good standing) in the state of practice
• At least 1 year of recent home health experience in a field setting
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Exceptional clinical assessment, communication, and organizational skills
• Reliable transportation, valid drivers license, and active auto insurance
• Ability to operate independently and manage a flexible schedule

Preferred:

• BSN degree
• Experience managing caseloads of 2532 points per week
• Familiarity with Medicare and home health compliance standards
