Job Description

Deliver specialized, compassionate care for patients at a pivotal time in their lives as a Hospice Registered Nurse. This contract opportunity calls for an experienced RN dedicated to delivering outstanding home-based hospice and end-of-life support. The position centers on guiding patients and families through the hospice journey, ensuring comfort, dignity, and high-quality care tailored to each individual's needs.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM (occasional weekend or on-call rotation may be required)
  • Approximately 45 patient visits per day
  • Field-based, with travel between homes, assisted living, and nursing facilities

Key Responsibilities:

  • Perform thorough nursing assessments and develop individualized care plans
  • Deliver skilled symptom management, pain mitigation, and patient-centered interventions
  • Administer medications and treatments under hospice protocols and physician guidance
  • Monitor patient status, promptly communicating changes to the care team and updating plans as needed
  • Educate patients, families, and caregivers regarding comfort care, medication, and disease progression
  • Work collaboratively with social workers, chaplains, aides, and volunteers to deliver holistic care
  • Ensure accurate and timely documentation in the Home Care Home Base (HCHB) platform
  • Maintain compliance with all hospice regulations and participate in team conferences and quality initiatives

Qualifications:

  • Active RN license in good standing
  • At least 1 year of recent hospice nursing experience
  • Proficiency with Home Care Home Base (HCHB) required
  • Strong clinical, communication, and assessment skills
  • Reliable transportation, valid drivers license, and auto insurance
  • Ability to work independently in a field-based role
  • Compassionate, resilient, and mission-driven

Preferred:

  • BSN
  • Experience in palliative or end-of-life settings
  • Familiarity with Medicare hospice regulations/documentation
