An excellent opportunity is available for a skilled and compassionate Registered Nurse with a background in home health. As a Home Health RN, you will play a vital role in delivering high-quality, patient-centered nursing care directly in patients homes. This full-time, 13-week contract offers a dynamic and rewarding work environment where you will collaborate with interdisciplinary teams and support patients in reaching their highest potential.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based visits to patient homes
• Daily travel within an assigned territory (mileage reimbursement provided)
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Electronic documentation using Home Care Home Base (HCHB)
• OASIS utilized for assessments

Key Responsibilities:

• Conduct thorough OASIS assessments and coordinate individualized patient care plans.
• Deliver a range of skilled nursing interventions including wound care, IV therapy, medication management, and chronic disease support.
• Communicate effectively with physicians and care teams to adjust care as patient conditions evolve.
• Educate patients and families on health plans, disease management, medications, and preventive care.
• Ensure timely, accurate documentation of patient care in HCHB.
• Collaborate with physical/occupational therapists, medical social workers, and home health aides.
• Participate in staff meetings, case conferences, and ongoing education.
• Follow all regulatory and agency documentation and care standards.

Qualifications:

• Active RN license (in good standing)
• 1+ year of recent home health field experience
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS
• Valid drivers license, reliable transportation, and current auto insurance
• Strong assessment, critical thinking, and organizational skills
• BSN and Medicare/home health compliance knowledge preferred

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
