Job Description

A Home Health Registered Nurse (RN) position is available for a skilled and compassionate nursing professional. This full-time, 13-week contract role involves providing high-quality, patient-centered skilled nursing care in patients homes. The ideal candidate demonstrates strong clinical skills, works independently, and delivers effective communication and coordination with interdisciplinary healthcare teams.

Responsibilities include conducting comprehensive OASIS assessments, developing and managing individualized care plans, and providing direct nursing care such as wound care, medication administration, IV therapy, and chronic disease management. You will monitor patient progress, educate patients and families, maintain accurate electronic documentation using Home Care Home Base (HCHB), and collaborate with therapists and home health aides to ensure coordinated care. Compliance with regulatory and agency standards is essential.

Qualifications:

  • Active RN license in good standing for the state of practice
  • Minimum of 1 year recent home health nursing experience, preferably field-based
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments
  • Strong clinical assessment, critical thinking, and organizational skills
  • Reliable transportation, valid drivers license, and auto insurance
  • Ability to work independently and manage a flexible field-based schedule Preferred:
  • BSN degree
  • Experience managing 2532 points per week caseload
  • Knowledge of Medicare and home health compliance requirements

Schedule & Location:

  • Monday through Friday, 8:00 AM to 5:00 PM
  • Field-based, with daily travel to patient homes within assigned territory (mileage reimbursed)
  • Caseload expectation: 2532 points/week, typically 35 patient visits per day
