An opportunity is available for a Home Health Registered Nurse to provide exceptional patient-centered care in a field-based setting. This contract role focuses on helping patients maintain optimal health and independence in their homes, requiring a blend of clinical skill, autonomy, and a compassionate approach.

Schedule:

• Monday Friday
• 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress, monitor for changes in condition, and promptly communicate with physicians and care teams.
• Educate patients and their families on disease management, medications, and preventive strategies.
• Accurately document care and assessments using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides for holistic patient care.
• Participate in interdisciplinary case conferences, staff meetings, and required training.
• Adhere to regulatory and agency standards for care delivery and documentation.

Qualifications:

• Active RN license (state of practice).
• Minimum one year of recent home health experience (field-based preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Strong clinical assessment, communication, and time management skills.
• Reliable transportation, a valid drivers license, and proof of auto insurance.
• Ability to work independently and efficiently manage a caseload of 2532 points per week.

Preferred Qualifications:

• BSN.
• Previous experience managing higher caseloads and familiarity with Medicare home health standards.
