Job Description
Deliver high-quality, patient-centered care as a Home Health Registered Nurse providing skilled nursing services in patients homes. This 13-week contract role offers the opportunity to work autonomously while collaborating with an interdisciplinary healthcare team to ensure optimal patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
- Evaluate patient progress and notify the care team of any changes in condition.
- Educate patients and families on disease management, home safety, and prevention strategies.
- Maintain thorough and timely documentation using Home Care Home Base (HCHB).
- Coordinate with physical therapists, occupational therapists, social workers, and home health aides.
- Engage in case conferences, staff meetings, and ongoing education.
- Adhere strictly to all regulatory, professional, and agency guidelines.
Qualifications:
- Active RN license in good standing for the state of practice.
- At least 1 year of recent home health (field) experience.
- Proficiency in OASIS documentation and Home Care Home Base (HCHB).
- Strong assessment and clinical decision-making skills.
- Reliable transportation, valid drivers license, and current auto insurance.
- Ability to manage a flexible field-based schedule independently.
- Excellent time management, communication, and organizational skills.
- Preferred: BSN, prior management of 2532 weekly points, knowledge of Medicare and home health compliance.
