Deliver high-quality, patient-centered care as a Home Health Registered Nurse providing skilled nursing services in patients homes. This 13-week contract role offers the opportunity to work autonomously while collaborating with an interdisciplinary healthcare team to ensure optimal patient outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress and notify the care team of any changes in condition.
• Educate patients and families on disease management, home safety, and prevention strategies.
• Maintain thorough and timely documentation using Home Care Home Base (HCHB).
• Coordinate with physical therapists, occupational therapists, social workers, and home health aides.
• Engage in case conferences, staff meetings, and ongoing education.
• Adhere strictly to all regulatory, professional, and agency guidelines.

Qualifications:

• Active RN license in good standing for the state of practice.
• At least 1 year of recent home health (field) experience.
• Proficiency in OASIS documentation and Home Care Home Base (HCHB).
• Strong assessment and clinical decision-making skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to manage a flexible field-based schedule independently.
• Excellent time management, communication, and organizational skills.
• Preferred: BSN, prior management of 2532 weekly points, knowledge of Medicare and home health compliance.
