Deliver compassionate, patient-focused care as a Home Health Registered Nurse (RN) providing in-home services. This vital contract position specializes in home health, prioritizing high-quality care, clear patient communication, and collaboration with interdisciplinary teams.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Evaluate patient condition, monitor for changes, and communicate timely updates with providers and team members.
• Educate patients and families on disease processes, medications, and healthy living strategies.
• Maintain thorough, accurate documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides for coordinated care.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Uphold all regulatory and agency standards in documentation and direct care.

Qualifications and Preferred Background:

• Active RN license in good standing within the state
• At least 1 year of recent home health experience (field experience highly valued)
• Proficient in Home Care Home Base (HCHB) and skilled with OASIS
• Sound clinical judgment, strong organizational skills, and effective communication
• Reliable transportation, valid drivers license, and auto insurance
• Ability to thrive independently while managing a field-based, flexible schedule
• BSN and previous experience managing a caseload of 2532 points per week desired
• Working knowledge of Medicare and compliance standards for home health
