Job Description
An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care within patients' homes, focusing on quality, continuity, and patient-centered services. This full-time, 13-week contract role is ideal for a self-motivated and organized nurse comfortable with independent work and maintaining clinical excellence.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop personalized care plans.
- Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, detect changes in condition, and communicate updates promptly to care teams and physicians.
- Educate patients and families on disease processes, treatment plans, medications, and preventive care.
- Maintain accurate and timely documentation using the Home Care Home Base (HCHB) system.
- Collaborate with therapists, social workers, and home health aides for coordinated care delivery.
- Participate in case conferences, staff meetings, and continuing education.
- Comply with all regulatory and agency documentation and care standards.
Qualifications and Desired Experience:
- Active RN license in good standing in the state of practice.
- Minimum of 1 year recent home health experience; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS.
- Strong clinical assessment and critical thinking abilities.
- Reliable transportation with a valid drivers license and auto insurance.
- Effective communication, time management, and organizational skills.
- BSN preferred.
- Prior management of a caseload of 2532 points per week.
- Knowledge of Medicare and home health compliance standards.
Schedule/Hours:
- Monday to Friday, 8:00 AM to 5:00 PM
- Caseload of 2532 points weekly (approximately 35 patient visits per day)
- Field-based role requiring daily travel within assigned territory (mileage reimbursement provided).
