Job Description
An experienced and compassionate Registered Nurse is needed to provide skilled nursing care within patients homes, focusing on quality, continuity, and patient-centered service. This home health role requires self-motivation, strong clinical skills, and the ability to work independently while maintaining excellent communication with patients and interdisciplinary healthcare teams.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize changes in condition, and communicate effectively with physicians and care teams.
- Educate patients and families about diseases, treatments, medications, and preventive care.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB).
- Coordinate care with therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing education.
- Comply with regulatory and agency standards for patient care and documentation.
Qualifications:
- Active RN license in good standing for the practicing state.
- Minimum of 1 year recent home health nursing experience; field experience preferred.
- Proficiency in Home Care Home Base (HCHB) and OASIS required.
- Strong clinical assessment and critical thinking abilities.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to manage a flexible schedule and work independently.
- Excellent organizational, communication, and time management skills.
Preferred:
- BSN degree.
- Experience managing 2532 points per week caseload.
- Knowledge of Medicare regulations and home health compliance standards.
Schedule & Location:
- Full-time, 13-week contract.
- Monday through Friday, 8:00 AM 5:00 PM.
- Caseload of approximately 2532 points per week (35 patient visits daily).
- Field-based practice traveling to patient homes with mileage reimbursement.
