Job Description

A skilled and compassionate Home Health Registered Nurse (RN) is needed to deliver exceptional in-home nursing care, ensuring patient-centered treatment and continuity. This role demands self-motivation, strong organizational skills, and independence while upholding clinical excellence and effective patient communication.

Key Responsibilities:

  • Conduct thorough OASIS assessments and develop personalized care plans.
  • Provide skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease oversight.
  • Monitor patient progress, detect changes in condition, and promptly communicate with physicians and care teams.
  • Educate patients and families about diseases, treatments, medications, and preventive care.
  • Maintain accurate, timely documentation in the Home Care Home Base (HCHB) electronic system.
  • Collaborate with interdisciplinary teams including physical therapists, occupational therapists, social workers, and aides.
  • Participate in case conferences, staff meetings, and continuing education.
  • Ensure compliance with regulatory and agency standards.

Qualifications:

  • Active Registered Nurse license in good standing.
  • Minimum one year recent Home Health experience; field experience preferred.
  • Proficient with Home Care Home Base (HCHB) and experienced in OASIS assessments.
  • Strong clinical assessment and critical thinking capabilities.
  • Reliable transportation, valid drivers license, and auto insurance required.
  • Ability to work independently with flexible scheduling.
  • Excellent communication, time management, and organizational skills.

Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Experience managing 2532 points weekly.
  • Familiarity with Medicare and home health compliance standards.

Job Details:

  • Contract position: full-time, 13 weeks.
  • Schedule: Monday through Friday, 8:00 AM to 5:00 PM.
  • Caseload: 2532 points/week (35 visits per day).
  • Field-based work within assigned territory; mileage reimbursement provided.
  • Documentation via Home Care Home Base (HCHB).
