Job Description
An exciting opportunity exists for an experienced and compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care to patients in their homes. This role is ideal for clinicians who are organized, self-motivated, and enjoy working independently while maintaining clinical excellence and effective patient communication.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based with daily travel to patient homes (mileage reimbursement provided)
- Caseload: 2532 points per week (typically 35 patient visits per day)
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management
- Monitor and evaluate patient progress, report changes promptly to the interdisciplinary team and physicians
- Educate patients and families on medication, diagnoses, care plans, and preventive strategies
- Accurately document all care delivered using Home Care Home Base (HCHB)
- Collaborate with therapists, medical social workers, and home health aides to ensure holistic, coordinated patient care
- Attend case conferences, staff meetings, and participate in continuing education
- Comply with all agency and regulatory standards for documentation and care delivery
Required Qualifications:
- Active RN license in good standing
- At least one year of recent Home Health nursing experience (field experience preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Strong clinical assessment, time management, and communication skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work independently and manage a dynamic field-based workload
Preferred:
- BSN degree
- Experience managing 2532 points per week
- Familiarity with Medicare/home health compliance standards
