Job Description

Experienced Home Health RN opportunity in Colfax, WA, offering a full-time, 13-week contract role focused on delivering skilled nursing care to patients in their homes. This position demands clinical excellence, strong communication skills, and the ability to work independently within a flexible field-based schedule.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and communicate effectively with the care team and physicians.
  • Educate patients and families about disease processes, treatment plans, medications, and preventive care.
  • Maintain accurate documentation using Home Care Home Base (HCHB).
  • Collaborate closely with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated care.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Adhere to all regulatory and agency standards for documentation and patient care.

Qualifications:

  • Active RN license in the state of practice (in good standing).
  • Minimum 1 year of recent home health experience; field experience preferred.
  • Proficiency in Home Care Home Base (HCHB) and experience with OASIS required.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Strong clinical assessment, critical thinking, communication, and organizational skills.
  • Ability to manage a flexible schedule and work independently. Preferred:
  • BSN degree.
  • Experience managing 2532 points per week.
  • Knowledge of Medicare and home health compliance standards.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (typically 35 patient visits daily)
  • Field-based work with daily travel to patient homes; mileage reimbursement provided.
