An opportunity is available for an experienced and compassionate Registered Nurse specializing in home health care. Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in good standing
• At least 1 year of recent home health nursing experience, with field experience preferred
• Proficiency with Home Care Home Base (HCHB) documentation system
• Prior experience with OASIS assessments
• Excellent clinical assessment and critical thinking skills
• Reliable transportation, a valid drivers license, and current auto insurance
• Ability to work independently, adapt to changing field-based schedules, and manage time efficiently
• Outstanding communication and organizational skills

Preferred Skills:

• Bachelors degree in Nursing (BSN)
• Familiarity with managing caseloads of 2532 points per week
• Knowledge of Medicare guidelines and home health compliance protocols

Primary Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions including wound care, IV therapy, medication management, and disease education
• Monitor, evaluate, and document patient progress; promptly communicate with providers and care team members
• Educate patients and families on their conditions and self-care strategies
• Collaborate with a multi-disciplinary team to ensure holistic and coordinated care
• Participate in case conferences, staff meetings, and ongoing professional development
• Ensure accuracy and regulatory compliance in all documentation through HCHB

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
